Function: Prepares reservations and issues tickets for all OCONUS travel (TDY and PCS) for all civilian, permanent party, students and their dependents. Processes passports and visa applications.
Anyone 20 and over must now have 2 forms of identification, including one photo I.D:

Military I.D. (Must not be expired)

State, federal, or local government I.D. card

Voter's Registration Card
Permanent resident card
Civilian contractors must be entered into the Contractor Verification System prior to receiving I.D. cards
Security badges are NOT acceptable forms of identification.
ACTIVE DUTY ID CARD
The following is needed for ID Card issue. No Exceptions.
Renewal/Expired Cards:
Soldier will need previously issued active duty ID Card
Extension or Reenlistment documents (if applicable)
Two forms of unexpired ID, as stated above.
Grade Change:
Promotion Order
MP Confiscation:
DA Form 4137 (Evidence of Property Custody Document)
Two forms of ID as stated above.
Lost/Stolen ID Cards:
Picture identification
Birth certificate or social security card
Ranks of E4 and below must have a counseling statement by their Supervisor/First Sergeant.
For the purpose of reissuance of a CAC to military, civilian and DoD contract personnel who are victims of stolen ID cards and have no form of picture ID: The second document must be an official report or letter from the Military Provost Marshal or Civilian Police Department indicating the CAC was stolen.
Eligible dependents of military sponsors whose military ID card has been lost or stolen, must obtain a valid photo ID and a second form of identification as proof of identity prior to reissuance of a new ID card.
CIVILIANS (Non-CAC card)
The following is needed for ID Card issue. No Exceptions.
All organizations whose employees are in need of a DA Form 1602 (non-CAC ID card) will maintain a current DD Form 577 (Signature Card) on file with In/Out Processing.
For contractors

RETIREE

USAF/NAVY/USMC: Retirement order or DD Form 214
SPOUSE, CHILD UNDER 21 YRS OF AGE (RENEWAL)
The following is needed for ID Card issue. No Exceptions.
(card can be renewed within 90 days of expiration)
Children must be 10 years of age to be issued an identification card with the exception of dual military and single military service members' children.
DD Form 1172 (ID Card Application) or the spouse must have the sponsor's Power of Attorney to sign for the sponsor in his/her absence or the spouse must have a DD Form 1172 not more than 90 days old signed by the sponsor and his/her servicing ID Card/DEERS Office.
INITIAL ISSUE (SPOUSE/CHILD):

Marriage certificate
Birth Certificate

ARMY RESERVE/NATIONAL GUARD (PRIOR SERVICE)
The following is needed for ID Card issue. No Exceptions.
Two forms of ID
DD Form 214 can be used if it reflects the reserve obligation date.
100% DISABLED VETERAN
The following is needed for ID Card issue. No Exceptions:
Require a letter from the Veterans Administration stating he/she is 100 percent disabled service-connected.
The letter must state whether the DAV is scheduled for future exams.

INDIVIDUAL READY RESERVE

CIVILIAN CONTRACTOR

CHILD, AGE 21-23
The following is needed for ID Card issue. No Exceptions:
Must be enrolled in an accredited college as a full-time student leading to an associates degree or higher
Requires a letter from the school Registrar's office certifying full-time enrollment and anticipated date of graduation and must be on school letterhead
Two forms of required ID as listed above. If ID card is lost or stolen, you must bring the letter from the school.
INCAPACITATED CHILD OVER AGE 21, PARENT/PARENT-IN-LAW DEPENDENCY
The following is needed for ID Card issue. No Exceptions:
Dependency determination can be done for a child 21 years of age who is incapacitated; parents, and parent-in-laws, provided the sponsor is supporting these dependents with at least 50% of their living expenses.
All packets must be renewed every 4 years or as required by the approving authority.
INCAPACITATED CHILD 21 YEARS OF AGE
The following is needed for ID Card issue. No Exceptions:
DD Form 137-5
Birth certificate
Medical statement verifying the dependent's medical incapacitation is permanent or temporary
Retirement order (if sponsor is retired)
Letter from the Social Security office verifying whether the child is receiving social security benefits, and if so, whose social security number the benefits are being received under.
PARENT/PARENT-IN-LAW
The following is needed for ID Card issue. No Exceptions:
DD Form 137-3
Sponsor's birth certificate, if it is the sponsor's parent, spouse's birth certificate and marriage certificate, if the spouse's parent. If the parent has medical problems, a medical statement is required.
If the sponsor is retired, retirement order or DD Form 214 is required in addition to the other document listed.
NEWBORNS
The following is needed for ID Card issue. No Exceptions:
All newborns must be added to DEERS within 30 days of birth.

Single male service members should contact the ID Card Office for required documentation.
